I take it you're trying to fake a doctor's note? And I don't think the type of paper matters. If they use a blank sheet of paper it might not be as
believable as one with their official heading on it.

It's not out of the realm of possibility they'd use a blank sheet of paper, but I wouldn't believe one.

If they use a blank sheet of paper (very uncommon!), the note should include some other means of identification (their stamp for example).
