You need to change your life. Start by going to doctor to get a physical and a weight plan.

Set manageable goals for yourself and work toward them. Get out and do things you enjoy. Don't just sit in front of the TV or computer every night.
Develop self-confidence. Stop thinking negative thoughts. Tell yourself that you are awesome. Because you are. Everyone is awesome in their own special way.
